Errors of Direction
This type of error consists of aiming the HPT effort at the wrong goals in the first place, such as trying to improve performance that would not add sufficient value to the organization even if it were improved, or making a mistake about the presumed business value of an apparent business goal or need.

Errors of Analysis
This type of error creeps in after the HPT practitioner has decided on a goal to address but for one reason or another fails to accurately identify the root causes of the performance issue.

Errors of Design
Assuming a valid performance issue and the correct analysis of causes, the next opportunity for error is with the design of a performance-improvement solution.

Errors of Implementation
Assuming all is correct in goals, analysis, and designs, the success of the HPT venture now relies on thoughtful and responsive implementation. The old adage of the best-laid plans of mice and men applies; there can be many a slip twixt plan and action. The more complex the HPT ffort, the more likely it is that something will go wrong during implementation.

Errors of Sustained Impact
This type of error emerges at the point that all preceding work appears to have been done well. That is, the goal was correct, the analysis valid, the design workable, and the implementation faithful, but for some reason, performance just does not get adequately improved.
