Herewith I declare that Contractor have been briefed me regarding my deployment on Total E&P Indonesie, and I have been understand my job then I with my competencies are able to deliver the jobs as per required by Total E&P Indonesie.

I declare that Contractor have been briefed me with the information regarding Total E&P Indonesie that required during my deployment on Total E&P Indonesie premises including but not limited to Healthy, Safety and Environment aspect, Security aspect, Code of Conduct aspect, Prevention to the Corruption aspect, Communication Procedure and Administration aspect.

Thus this Acknowledgement Form is made, and the I understand clearly the contents, with good intention, full conscious as well as in good physical and mental conditions without interfering from any other parties.

Dengan ini saya menyatakan bahwa Kontraktor telah diberi pengarahan saya mengenai penyebaran saya di Total E & P Indonesie, dan saya telah mengerti pekerjaan saya maka saya dengan kompetensi saya mampu memberikan pekerjaan sesuai yang dibutuhkan oleh Total E & P Indonesie. Saya menyatakan bahwa Kontraktor telah diberitahu saya dengan informasi mengenai Total E & P Indonesie yang diperlukan selama penyebaran saya di Total E & P Indonesie tempat termasuk namun tidak terbatas pada Sehat, Keselamatan dan aspek lingkungan, aspek keamanan, Kode aspek Perilaku, Pencegahan pada aspek Korupsi, Prosedur Komunikasi dan aspek administrasi. Jadi Formulir Pengakuan ini dibuat, dan saya mengerti dengan jelas isi, dengan niat yang baik, kondisi fisik dan mental penuh sadar serta yang baik tanpa mengganggu dari pihak manapun.
